WOW
The Warriors of Wellness (WOW) Collaborative is a model created in 2014 through which healthcare organizations can contract for culturally-specific community health worker (CHW) services through local community-based organizations (CBOs) to improve the health of their members while improving health equity. Culturally specific CHW program services include one-on-one support and health education, health care navigation, community organizing, group classes, and cultural mediation among other services.
WAH
The project expands support for immigrant and refugee families by incubating community-driven, culturally specific approaches to maternal, child, and family health services.
Through this collaborative, immigrant and refugee-led organizations working with communities who are underserved by the current early childhood service array leverage their own community wisdom and assets to strengthen community connection and cultural preservation, assist individual families navigate complex systems, and are pursuing systems change by identifying barriers and opportunities to better partner with immigrant and refugee parents.
KAISER
The proposed project supports AFHHO in the categories of Operations & Strategy, and People and Organizational Culture. AFHHO develops and sustains the organization by building the capacity of staff and board members in fundraising, project management, community organizing, and communications. The improved fundraising allows AFHHO to sustain and enhances the important early learning activities, education, and healthcare projects we provide to the community. The staff and volunteer board members enhance their skills in program coordination, community networking/ organizing, and fundraising strategy development.
